Position Summary

The University of Arizona is hiring two Coordinator, Patient Navigators to work on a care coordination intervention with cancer patients and survivors. This will be a position with both in-person and telephone interaction with patients being treated at the University of Arizona Cancer Center (UACC) and survivors who have completed treatment. This position is funded through an award from the Merck Foundation. Individuals who can work full time are preferred, but part time employment is possible.

Goals of the position include optimizing the experience of patients and their families who receive oncology care at UACC, in addition to helping patients navigate healthcare after oncology treatment completion (cancer survivorship). Patient navigators will effectively interface and collaborate with providers and staff to identify solutions to barriers to care and care coordination, involving any and all team members as needed. Primary supervisors include Drs. Leila Ali-Akbarian, Beth Calhoun, Julie Armin, and Heidi Hamann. This position will start in Spring 2018 with potential opportunity to continue through Fall 2021.

The navigators will need to obtain research privileges associated with work at the cancer center.

Duties & Responsibilities

1. Make contact and form navigation alliance with cancer center patients that have been pre-determined by providers and/or staff to have high levels of distress, or who are underserved or under-resourced in other ways.

2. Examine patient and/or staff concerns in order to identify needs and barriers to healthcare, help provide resolution to patient issues or assistance and support as appropriate, in collaboration with all team members.

3. Participation in pre-determined training activities to better serve oncology patient population in addressing needs or barriers to care.

4. Maintain accurate and timely log of patient encounters, including types of assistance or support provided, as well as other data collection as requested.

3. Support the research team in terms of working towards strategic goals involving lay navigation services, including flexibility to adapt to unforeseen barriers or issues.

4. Provide and maintain effective communications with all constituents. This will include participation in meetings as requested.

5. Collaborate with social services, nurse navigators and community health workers, as appropriate, to offer resources such as lodging and transportation, financial assistance, and help with coordinating and organizing healthcare appointments

6. Coordination of patient care, after the completion of oncology care, with primary care and other healthcare services as needed

7. Meet and accompany patients and/or family to medical appointments or social service visits as appropriate

8. Exhibit awareness of and respect for cultural differences and diversity.

9. Maintain professional boundaries with patients and families, including restraint in terms of providing medical or social advice that is outside of the scope of lay navigation services.

10. Perform all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.

Knowledge, Skills, & Abilities
Minimum Qualifications

Proficient use and understanding of medical terminology.
Fluency in Spanish.
Excellent time management and problem-solving skills, ability to meet constantly shifting deadlines/timelines, and excellent patient interaction skills.

Preferred Qualifications

Medical Assistant Training, Patient Care Technician or Certified Nursing Assistant Training.
Two years of health care experience.
